(NCQA). NCQA is a private, non-profit organization dedicated to
improving health care quality everywhere. NCQA is the nation's leading
accreditor of a wide range of health care organizations, including HMOs. NCQA
also manages the evolution of HEDIS® , the performance measurement tool
used by more than 90 percent of the nation's health plans. For more information

National Heart, Lung, and Blood Institute
(NHLBI). NHLBI, part of the Federal Government's National Institutes
of Health, provides leadership on research and treatment issues related to
diseases of the heart, blood vessels, lungs, and blood. The Institute plans,
conducts, fosters, and supports an integrated program of basic research,
clinical investigations and trials, observational studies, and demonstration
and education projects. NHLBI also coordinates national education programs such
as the NCEP, which develop guidelines and promote education of professionals,
