Performance Goal #1 Promote Asthma Awareness Objective Increase the percentage of children and adults who are aware of the signs and symptoms of undiagnosed asthma and who seek care if they experience them. Performance Goal #2 Diagnose and Manage Asthma Objective Increase the percentage of patients with asthma who receive appropriate diagnosis and treatment. Performance Goal #3 Implement Community Asthma Programs and Policies Objective Increase the percentage of schools, churches, childcare centers, and other organizations that implement asthma-friendly programs and policies. Performance Goal #4 Prevent Recurrent Exacerbations of Asthma Objective Reduce the percentage of patients with asthma who require repeated emergency and/or urgent care visits or hospitalizations due to uncontrolled asthma. |
